"BlueCross BlueShield" isn't a single insurer — it's a federation of more than 30 independent, locally operated member companies, each with its own medical policies. If your plan card says CareFirst, that's the BlueCross BlueShield licensee for Maryland, D.C., and Northern Virginia — see our CareFirst page for coverage details specific to our region. If you're covered through a different state's Blue plan (for example, if you moved here or work for an out-of-state employer), your exact TMS and Spravato® coverage rules may differ slightly from what's below, even though the general pattern is similar across most Blue plans.

TMS Coverage Under Most Blue Plans

Across most BlueCross BlueShield licensees, TMS is covered for major depressive disorder (MDD) when medical necessity criteria are met.

Most Blue plans require a confirmed diagnosis of moderate-to-severe MDD, along with documentation that one or more antidepressant trials didn't provide adequate relief or weren't tolerated.

Nearly all Blue plans require prior authorization before TMS treatment begins. Our team can help submit the documentation your specific plan requires.

A standard initial course is typically 5 days a week for around 6 weeks, followed by a taper — though exact session counts and taper schedules vary by plan.

Your specific out-of-pocket cost depends entirely on your local Blue plan. We can help you confirm your benefits before your first session.

Spravato® Coverage Under Most Blue Plans

Most Blue plans cover Spravato® (esketamine) for treatment-resistant depression once prior authorization and clinical criteria are met.

Most plans require you to be 18 or older, with treatment-resistant depression or MDD with suicidal ideation, and documentation that two or more antidepressants from different classes didn't adequately relieve your symptoms.

Prior authorization is required by every Blue plan we've worked with for Spravato®, typically including your diagnosis, medication history, and clinic certification.

Spravato® can only be administered at a clinic certified under the FDA's REMS program, regardless of which Blue plan you have. Led Life LLC is a certified authorized SPRAVATO® provider.
