For MDD, Cigna requires a diagnosis of moderate-to-severe depression without psychosis, along with documentation that two or more antidepressants from different drug classes — each tried for at least four weeks at a therapeutic dose — didn't provide relief (or weren't tolerated).

Cigna has moved to reduce prior authorization requirements for TMS in many plans — but requirements can still vary by plan, so we confirm this for you before scheduling.

An initial course typically covers 30–36 sessions. Repeat courses may be covered after a relapse if your prior course achieved at least 50% improvement that lasted two months or longer.

You'll generally need to be 18 or older, with a confirmed diagnosis, and documentation of an inadequate response (25% or less symptom improvement) to at least two antidepressants from different classes, each used at a therapeutic dose for a minimum of six weeks.

Prior authorization is required, and Spravato® must be prescribed by, or in consultation with, a psychiatrist. Initial approval is typically granted for 6 months.

Spravato® can only be administered at a clinic certified under the FDA's REMS program. Led Life LLC is a certified authorized SPRAVATO® provider.
